You need to stop applying capitalist concept of "wage" and "labour" to the idea of communist society.
Communist ideology assumes that the doctor, scientist and construction worker perform their jobs based on their mental and physical predispositions to do their job for the wellbeing of the community, not for the drive for wealth.
If you have all your needs covered in a ideological communist society what would you need "wages" for?
The doctor works by providing healthcare to the farmer. The farmer works so the doctor provides him with healthcare. The construction worker works so the farmer and the doctor have workplaces to to their work. He in return works for the food and healthcare the former provide.
The scientist conducts his research in a facility built by the construction worker, fed by the farmer and kept in good health by the doctor so the farmer, construction worker and doctor can work less and be more productive. So on and so forth, no idea how to put that in simpler words.

There's a reason Marx believed only a developed and already industrialized country/nation can become "communist". The purpose of communism as opposed to capitalism is not perpetual growth for god knows what fucking reason or purpose by endlessly exploiting natural and human resources to build up personal wealth but stabilization and making sure everyone is getting their share of the pie everyone worked for to bake.
We all know this idea was and still probably be a failure for centuries for many reasons but "duhhhh construction worker and doctor can't earn the same!!!" is truly a low iq argument.
