For me visually not so much close to death, but just a woman with a really skinny frame with a (to me) unhealthy minimum diet really.

Although I can feel the potential of her having a lot of warmth,
to me it's either because she's too scared of letting go the direct control over the gem empire, because she lost pink and is now so deep into the escapism from that reality, that she cannot let go, even though Pink IS back and was right in front of her.
That or White Diamond started out as the Super-Computer that controlled the expansion of the gem empire(being capable of moving around), which progressed so far from the original size that allowed her to still move, that she has become stuck in a loop that prohibits her from functioning independently.
