Think about the methods, though. Compare the way he tried to save Mikoto or handled the aluminum bomber incident and compare it to how he handles, say, saving Orsola. When he aimed to save Mikoto from the thugs, he tried first to avoid any conflict by pretending to be with her and pulling her away before they could react properly, because he was actually pretty scared of getting hurt (like a normal person would). Compare that to Orsola's case where he kicked down the door, walked up to Agnese and tried to punch her in the face. He was planning to take the loli with him and get the fuck out of there but he was stuck with an immminent explosion on his hands. Post-amnesia Touma would have told Mikoto to take the kid and get out while he looked for the criminal.

You starting to see the difference yet? Post-amnesia Touma always seems to take on all the responsibility and doesn't care about his own life while pre-amnesia Touma specialised in running away from the problems he was in because he didn't like getting hurt but he'd fight if he was backed into a corner. The first time he'd ever done anything on post-amnesia Touma's level was when he made the decision not to run from the giant fire monster so he could call Anti-Skill and instead made Index's problem his problem and faced it head-on in order to save someone her.

That might simply be because pre-amnesia Touma was involved in any incident similar to amnesiac Touma that he was emotionally involved in. This is merely a theory though, since there are very few details about Touma's past life. I say that he always had it in him, but he rarely has the chance to display it.

I think that he does care about his own life. It's just that in the incidents that he has been involved in, there were things that couldn't be abandoned so easily.

To be more accurate, it isn't that he didn't have the chances, he just couldn't bring himself to do it. He had a family, he had a school life, he had friends at that point - he had too much to lose to be a full blown hero and gamble his life.

Maybe this is a better contrast to Orsola. Touma had seen Index bleeding out on the floor outside his apartment and yet he STILL almost ran away to "call Anti-Skill" and had to seriously think about whether or not risking his life to save her was a good idea. Touma knew Orsola for less time than he had known Index before he decided to help her, was told repeatedly by everybody around him that going to save Orsola would be suicide, went with the assumption that he'd be up against a force of over 200 all by himself, hadn't actually seen the way she'd been treated by Agnese, and was told that saving her would give the Catholic Church a reason to attack the Anglican Church out of retaliation - and yet he still didn't even think twice about going off to save her.
