Maki obviously has some mental obstacle with Kizuna being male and doesn't want to see the latter's penis as to not be reminded of this and he's probably just suppressing this knowledge. He boldly proclaimed “Your gender doesn't matter.”, but he spent the entire title treating it as an obstacle so I'd doubt those words.
Kizuna is 16 at this point with a good chunk of puberty still ahead of him so he'll probably develop and masculinize further, but even assuming that that not be the case, Maki is going to have a hard time when they finally have sex and he sees his naked body, and he'll have to accept that Kizuna is indeed male.
Sazunami Cherry at least acknowledged this problem and ended on an interesting ending that acknowledged that it might actually be problematic for their relationship in the future as the trap completes his puberty, but that for the time being they enjoy their relationship.
