Fighting a ritualized duel over something is a thing that happened frequently in real life. And besides, the duels and the revues are different. Revue places a much heavier emphasis on personal antagonisms and rivalries. The goal of the revues is to plunder each other's brilliance, whereas the goal of the duels is a nebulous "revolution". Both Utena and Karen join out of a desire to preserve a relationship, but the nature of this relationship is not the same.
