The Annihilation route and the after-credits scene of Ota shows that by that point she was part of the body hopping chain (to be able to switch the Killer needs someone to be missing an eye, so he removes one from the body he is currently residing and after the switch is done he then kills the previous body)
Which means that even though Ota stopped Iris body from being killed, the real one is dead (when you're questioning Iris, the one you're talking to is the killer), leaving the Old man in her body as he was the next one in the chain
