You sign-up for it, alike gmail. You can use it with an e-mail client program and don't have to do a web sign-in. And it's for free-price, gratis.
danwin1210.me is another alternative, but it was down, maybe up. I think
cock.li may be down now, or can't seem to register new accounts, right now. Other alternatives are
yandex.com is like g-mail, and works with e-mail client programs, if you don't mind web-sign in, or it being russian.
Tutanota.com is web-based only and says you can only sign up for 1 account, but you can seem to sign up for a lot, they don't enforce the rule, probably unless you're like an actual spammer spam-bot user person.
Protonmail.com is also good but web-based only. There may or may not be work-arounds, but it doesn't straight-up work, or "just werks", as it were.
